Reservation Forms

Throughout the year, student organizations will be required to submit forms with the Center for Student Involvement to remain active. Also, there are other forms that can be filled out so that the organization can reserve rooms on campus, banner space and table tents in the cafeterias. The following links provide you with a synopsis of these forms and where student organizations can find them.
Center for Student Involvement Mailbox Forms
Student organizations must be registered through the Center for Student Involvement. Throughout the year, the Center for
Student Involvement will ask organizations to fill out forms pertaining mostly to an updated officer list. For an organization to
have a mailbox in the Center for Student Involvement, it must have an updated officer form on file. These forms are
distributed once a semester in each organization’s mailbox located in the Center for Student Involvement.
Room Reservations
Student organizations are allowed to reserve rooms on campus for activities and meetings. Reservations can be made online
at Union Reserve website or by stopping by the main office in the Student Union, located on the main floor.
The Student Union can reserve rooms in the Student Union and the academic buildings on campus, as well as the mall area
outside the Student Union. Outside venues, such as the Quad, Sunken Garden and Red Barn Park can also be reserved
in the Center for Student Involvement. Once a semester, packets are sent to student organizations by the Student Union
Office for the meeting room lottery. The lottery is used to reserve meeting rooms for organizations for the entire semester.
Entering the lottery does not guarantee that the organization will receive the room and time slot they desire. However, not
entering the lottery will make it more difficult for the organization to reserve a room for the entire semester for a meeting.
Also, if the organization knows of individual events that it plans to hold over the course of the semester, they can include
these in the lottery as well.
Banner Space
The Center for Student Involvement reserves banner space located on McClain Hall and Pickler Library. There are five spaced
on McClain and 17 on Pickler. A student organization must have completed the banner before reserving the space. Someone
from the organization needs to fill out a form in the Center for Student Involvement, present the banner to an
office worker, and then deliver the banner to the Physical Plant Office in the Kirk Building. Also, the student organization
must provide at least 50 feet of rope to hang the banner.
Table Tents
Student organizations may also reserve the tables in the residence hall cafeterias and Mainstreet Market to publicize with
table tents. The table tent must be presented to the Food Service Office located in the Student Union Main Office. The
Food Service Office reserves the table space and regulates the table tents.
